Online & Telephone Counseling Terms and Conditions Of Use
Prior to starting online and phone counseling and therapy, you acknowledge that you have read, comply and agree with the below conditions and terms where "I" refers to you (the client) and "we" refers to Mainland Counselling Services, Wayne Shoblom, and associates.
You are agreeing that you are engaging in counseling and therapy with Wayne Shoblom, MSW, RCSW (Registered Clinical Social Worker) and associates of Mainland Counselling Services in the jurisdiction of British Columbia, Canada.
Informed Consent
I acknowledge that I am receiving professional counselling services administered by Mainland Counselling Services (and associates) and whereby I am receiving counselling and therapy services under which Wayne Shoblom and associates provide to the extent that they are qualified to provide in the jurisdiction of British Columbia, Canada.
I acknowledge that these services are limited, cannot provide assessment for clinical diagnostic purposes due to the lack of face-to-face contact, and these services do not replace possible need for in-office sessions.There are several areas of human behaviour that cannot be assessed online or on the phone, much of which relate to assessing body language cues, facial language cues, level of distress relating to in-office observations, contact with your local health care professionals, family and friends, etc. By proceeding to the payment page, I understand and agree that I am giving informed consent to receive the services provided by Wayne Shoblom, Mainland Counselling Services director and associates.
I hereby agree that I am representing myself truthfully and accurately to the best of my knowledge.
I acknowledge that I may stop the process of counselling at any time with Wayne Shoblom or Mainland Counselling Services and associates. I also acknowledge that the 100 % Satisfaction Guarantee only means that if I am not satisfied with the counselling services I receive for any reason, I will receive 100 % of my money back (less $ 10.00 for handling fees) for one e-mail consultation cost or one phone counselling service. I understand that I must request this refund in 30 days or less of the last or recent attended phone counselling appointment or e-mail consultation.
I agree that for phone counselling services, I must provide 24 hours notice to Wayne Shoblom and associates if I cannot make my agreed upon appointment time. If I do not for any reason attend my phone counselling session, I acknowledge that Wayne Shoblom and Mainland Counselling Services and associates will charge me 100 % of fees paid for the unattended appointment and I am not entitled to any monetary refund for this session for any reason.
Confidentiality and Privacy
There are a few limits to confidentiality. Wayne Shoblom and associates must report child abuse to the local child abuse authority. We also must report to the police if you are going to harm yourself or others and we must warn the target person(s) if you report threatened harm to another person(s). We must respond to any supoenas in the court of law and must produce any documentation that we have relating to you and our work together. We do write an initial brief assessment and follow-up session(s) data and keep them on our computer(s) and in a file locked in our local offices located in or near Surrey, British Columbia.
Our work together is confidential and private, however, I understand that Online counseling and e-mail counseling does not guarantee confidentiality. This relates to the imperfections of transferring data through email, keeping data on computer systems, other people accessing the computer system you use (family, friends, employers, etc.) hackers, and other computer system imperfections. I understand that I must take certain privacy precautions to avoid some of these potential problems (printing our work and deleting it from your computer, setting up private passwords, using your own private computers, not using work or employer based systems, etc.). Regardless of how well I am protected, and regardless of the protections Mainland Counselling Services have, I understand that my information may get into the hands of others and I agree not to hold Mainland Counselling Services, Wayne Shoblom and associates responsible in any way (monetarily or otherwise) if my privacy is unknowingly compromised.
I understand that Mainland Counselling Services does not give away or rent your private email to any third party. From time to time, or regularly, however, we may send promotional, educational and other material to your email address.
Age
I agree that I am 19 years of age or older. I understand that I cannot participate in Online Counseling or telephone counseling without the consent of a legal guardian or parent if I am under 19 years of age.
Mental Health Status
I acknowledge and agree that I am not at risk of harming myself or others and that I do not have a severe mental health condition (i.e, schizophrenia, psychosis, etc.) that would prevent me from receiving counseling help. I also acknowledge that the online and phone counseling services cannot effectively respond to crises related to domestic violence, child protection needs, sexual assault or other kind of trauma needing face-to-face assistance. I acknowledge and agree that if I am in severe crisis like the situations mentioned above, I need to see a medical professional face-to-face (i.e, physician) or go to my local hospital immediately. Moreover, I agree to alert Wayne Shoblom and Mainland Counselling Services associates if I encounter any of these crises during the period of counselling services with us.
Responsibility
I acknowledge that I am responsible for any decisions or actions that I take as a result of the counseling and therapy services that I receive from Wayne Shoblom and associates of Mainland Counselling Services. We will do our utmost to provide you with sound professional counseling advice and services but phone and online counseling does not replace in-office or face-to-face counseling services and thus, we cannot be responsible in any way for any actions you take as a result of the e-counselling or phone counselling services.

I understand that I am releasing my personal information to Paypal Financial Services. This service is confidential and private.
* Phone counselling is not appropriate for people who are considering hurting themselves (suicidal) or others. We strongly recommend that if you have these thoughts, you see a local physician or to immediately go to your local hospital for mental health treatment. |
